1. Purpose of This Consent

This document explains how Longaevitas Health (Pty) Ltd ("Longaevitas Health," "we," "our," or "us") and your nominated healthcare provider (if any) use your personal and health information within the Longaevitas Health Platform to support proactive health, wellness, and longevity planning. By signing or digitally accepting this form, you authorise Longaevitas Health and, where applicable, your nominated doctor/practice to collect, process, and use your personal information (including special personal information/health information) for the purposes below.

2. Nature of the Services

The Longaevitas Health Platform integrates laboratory results, body composition scans, vitals, and lifestyle data to assist in generation of risk insights, trends, and personalised longevity plans. Other forms of medical data can also be uploaded and used by the platform.

These insights are decision-support tools intended to assist your clinician and you in understanding health risks.

They do not constitute medical advice or diagnosis on their own. All clinical decisions remain the responsibility of your treating healthcare professional.

3. Data Collected

Longaevitas Health and your nominated doctor/practice may collect and process the following data types (depending on what you provide or connect):

Identification and contact information.

Medical and health data (lab results, vitals, body composition, clinical notes).

Lifestyle data (nutrition, exercise, habits, wearables).

Appointment records and longevity plans.

Communications between you and your healthcare provider (where enabled).

Billing information necessary to administer your membership (note: payment card details are typically processed by third-party payment processors; we may receive confirmation and related transaction metadata).

    4. How Your Data Will Be Used

    Your data will be used to:

    Generate personalised health and longevity reports and decision-support outputs.
    Track trends in key biomarkers and health indicators over time.
    Facilitate structured consultations between you and your healthcare provider (if you nominate one).
    Enable population-level analytics in de-identified form to improve our system and outcomes research.
    Communicate with you regarding your results, follow-ups, service updates, or program updates.

    5. Data Access and Sharing

    Your identifiable data will only be accessible to:

    Your nominated healthcare provider(s) and authorised clinic staff (if you nominate one).

    Longaevitas Health technical and support teams (with access controls and confidentiality obligations) for system maintenance, support, and compliance.

    Accredited diagnostic laboratories or service providers (if applicable and authorised).

    Service providers supporting platform delivery (e.g., cloud hosting, payment processors), under appropriate data-processing agreements.

    Regulatory bodies where required by law.

    6. Cross-Border Data Storage

    Your data is securely hosted on cloud infrastructure (including AWS), which may be located outside South Africa. All transfers comply with Section 72 of POPIA, ensuring adequate protection equivalent to South African standards.

    7. Voluntary Participation and Withdrawal

    Participation in the Longaevitas Health program is voluntary. You may withdraw your consent at any time by notifying your provider or emailing support@longaevitashealth.com

    Withdrawal will not affect the lawfulness of processing that occurred prior to withdrawal but may limit our ability to provide the service.

    8. Risks and Limitations

    The Platform’s insights depend on data accuracy (lab results, scans, inputs).

    Errors or omissions in data may affect outputs.

    The Platform is not a diagnostic or emergency tool.

    Do not disregard or delay professional medical advice based on Platform insights.

    9. Your Rights (POPIA)

    You have the right to:

    Access your personal data.

    Request correction or deletion of inaccurate or outdated information (subject to legal retention).

    Object to the processing of your information in certain cases.

    Lodge a complaint with the Information Regulator of South Africa if you believe your data is mishandled.
